Palliative care may be provided at any point during cancer care, from diagnosis to the end of life. When a person receives palliative care, they may continue to receive cancer treatment.

Our Oncology Program can help prevent or relieve cancer symptoms. It also helps with treatment side effects. Hospice and palliative care help  you live your life as well as possible during this time.

Admissions Process

Contact Physician

With patient’s consent, we contact your physician to determine eligibility and criteria

Sign Consents

Upon approval, patient signs consent to be treated for hospice or palliative care service

In-Home Visit

An admissions nurse will schedule a visit the family and patient to discuss the plan of care
